Flu shots required by Nov. 15

To best protect the health of our patients and guests, our communities and everyone who works in our facilities, flu shots are required for all healthcare workers across our system.

Anyone who provides direct patient care or who plans to visit an Intermountain Health facility at least once during flu season is required to get a flu shot and submit proof of vaccination by Saturday, Nov. 15.

Flu shots are available for free at all Peaks Colorado Front Range hospitals during scheduled Flu Shot Clinics. You can view the schedule on MyIntermountain by clicking here.

For those unable to access MyIntermountain, all Front Range Flu Shot Clinics are listed below. You are invited to select the clinic most convenient to your home or work.

Submit your proof of vaccination

We offer several options to conveniently submit your proof-of-vaccination paperwork.

Employed medical staff members who receive a flu shot at an Intermountain clinic will have their paperwork submitted on their behalf by the clinic staff. Those who receive their flu shot outside of Intermountain – at a local pharmacy, for example – need to upload their documentation to the Employee Health Portal.

Non-employed medical staff members who receive their flu shot at an Intermountain clinic will need to fill out a Release of Information that allows the clinic to share your proof of vaccination with Medical Staff Services. Please click here to see what the ROI looks like. You will complete the form at the clinic.

Non-employed medical staff members who receive their flu shot outside of Intermountain Health, such as at a physician's office or local pharmacy, can submit proof of vaccination to us in one of the following ways:

  1. Option 1: Present your paperwork to your care site's Medical Staff Services office.

  2. Option 2: Email your documentation to flushots@imail.org and our CVO team will update the credentialing database.

  3. Option 3: Fax your documentation to Intermountain Health Medical Staff Services at 303-467-4284.

Flu shot exemptions

We recognize there are circumstances that would cause you to decline a flu shot. If you've already received an exemption from Intermountain Health, it remains valid; you do not need to reapply.

If you would like to apply for an exemption and have not previously done so, and you are employed by Intermountain Health, please click the button below and complete the form by Friday, Oct. 31.

If you are not employed by Intermountain Health, please secure an exemption directly from your own employer, group or medical organization by Friday, Oct. 31. You can submit your approved exemption documentation to Intermountain via any of the submission options listed above.

All practitioners who are granted an exemption will need to wear a mask at all times in Intermountain Health facilities during the flu season, which extends through March 31, 2026.
