Medication reconciliation within 24 hours is a new system KPI (key performance indicator), and a related KPI tracks completion of medication histories by pharmacy.
St. Mary’s pharmacy team has adjusted to prioritize medication histories, working to build the most accurate history possible with the information available. Gaps or conflicts in the medication history sometimes arise, depending on patient status or available records. In those situations, the pharmacy will send a Secure Chat message to the provider explaining the gap and will work diligently to resolve it in the upcoming days.
It's important to note that changes made to the home medication list after 24 hours affect both KPIs — neither pharmacy nor physicians/APPs receive credit for completing the med rec within 24 hours. However, a correct medication list remains the right thing to do for patient care. With that in mind, when the medication history status shows "RPh In Progress" or “RPh Complete,” please refrain from changing it to "Provider Complete." This allows the pharmacy team time to finish accuracy edits to the medication history.
