Rapid medication titration block charting soon to launch in Epic
Starting Friday, Aug. 1, rapid medication titration block charting will be used to capture the periods of time when rapid titration outside of standard written titration orders must occur to stabilize the patient.
A new process
In this new process, nurses create a note using SmartText that is then sent to the physician or APP for attestation, documenting the medical staff member was responsible for direction of patient care during the episode.
This is done after the patient is stabilized and rate changes are entered into Epic. The block of time can not exceed four hours.
A new note will be created for recurrent episodes of rapid medication titration.
Physicians and APPs must be made aware the patient condition requires rapid titration or medications outside the standard written parameters. Nurses should not deviate from written titration parameters without your consent.
Nurses are receiving information about the changes to their workflow. The physician and APP workflow is outlined below.
Physician and APP workflow
Once the patient is stabilized, you will receive an In Basket note for attestation and co-signature.
#1: From your In Basket, open the note to be cosigned.
#2: Click Attest.
#3: Find and add the attestation SmartText by searching RAPID in the Insert SmartText search field.
#4: Use the RAPIDTITRATIONBLOCKCHARTINGATTESTATION SmartText.
# 5: Sign the note after adding the attestation. The signed note with attestation is required for regulatory compliance.
