Upcoming and Recent Medication Class Harmonization

Some medication classes have not yet been harmonized in Epic; the process will continue for the next few months. Please use Epic as the best resource for what we currently carry during this transition time. 

These medication classes will be harmonized in Epic effective May 29: 

PAMORAs: No changes locally

Triptans
Sumatriptan subcutaneous solution removed

Dopamine agonists

·      Ropinirole 2 mg tablet removed

·      Added ropinirole 1 mg tablet

Azoles
Remove miconazole cream

These medication classes were harmonized effective April 30:

Statins
Simvastatin removed 

Fenofibrate  
Fenofibrate 48 mg and 145 mg were removed and replaced with 54 mg and 160 mg 

  • Doses 35 mg to 67 mg daily will be auto-substituted with fenofibrate 54 mg daily 

  • Doses 105 mg to 200 mg daily will be auto-substituted with fenofibrate 160 mg daily 

Tetracyclines
Doxycycline hyclate 100 mg tablet removed; doxycycline monohydrate 100 mg capsule still available 

ADHD Stimulants:
The following have been removed: 

  • Methylphenidate ER (Methylin) 10 mg tablet  

  • Dextroamphetamine (Dexedrin) 5 mg

As a reminder, the LMA will push the provider to the formulary agent, and for medications like ACEi, will include a table with equivalent dose, but it will not automatically populate in Epic. Double-check dosing during verification.
