Flu Vaccine Documentation Information
Proof of Flu Vaccination Due Nov. 15
All medical staff members who provide direct patient care or who plan to visit an Intermountain Health facility at least once during flu season are required to get a flu shot and submit proof of vaccination by Sunday, Nov. 15.
Employed Medical Staff
Employed medical staff members who receive a flu shot at an Intermountain clinic will have their paperwork submitted on their behalf by the clinic staff.
Those who receive their flu shot outside of Intermountain – at a local pharmacy, for example – need to upload their documentation to the Employee Health Portal.
Affiliated Medical Staff
Non-employed medical staff members who receive their flu shot at an Intermountain clinic will need to complete an electronic Release of Information form at the clinic, which allows the staff to share their proof of vaccination with Medical Staff Services.
You will be asked to scan a QR code and complete the ROI before you receive your shot. A confirmation email will be sent to the email address you provide.
Non-employed medical staff members who receive their flu shot outside of Intermountain Health, such as at a physician's office or local pharmacy, can submit proof of vaccination to us in one of the following ways:
Option 1: Present your paperwork to your care site's Medical Staff Services office.
Option 2: Email your documentation to flushots@imail.org and our CVO team will update the credentialing database.
Option 3: Fax your documentation to Intermountain Health Medical Staff Services at 303-467-4284.
Exemption Requests Due Oct. 31
We recognize there are circumstances that would cause you to decline a flu shot. If you've already received an exemption from Intermountain Health, it remains valid; you do not need to reapply.
Employed Medical Staff
If you would like to apply for an exemption and have not previously done so, please click here to complete the form by Saturday, Oct. 31.
Affiliated Medical Staff
If you are not employed by Intermountain Health, please secure an exemption directly from your own employer, group or medical organization by Saturday, Oct. 31. You can submit your approved exemption documentation to Intermountain via any of the submission options listed above.
All practitioners granted an exemption will need to wear a mask at all times in all of our facilities during the flu season, which extends through March 31, 2027.
