St. Mary’s Therapeutic Interchanges Updates

The following medication classes were harmonized at St. Mary’s this month, effective June 26.

Ophthalmics Removed:

There are many changes to what we currently carry. Please refer to Epic as the source of truth for what we carry.

  • Timolol 0.25% 

  • Timolol 0.5 GFS 

  • Brimonidine 0.1% 

  • Pilocarpine 2% 

  • Pilocarpine 4% 

  • Tobramycin 0.3% 

  • Tobramycin-dex 0.3%-0.1% oint  

  • bacitracin-neomycin-polymixin B ophthalmic ointment

  • NEOMYCIN 1.75 MG-POLYMYXIN 10,000 UNIT-GRAMICIDIN 0.025MG/ML EYE DROPS

  • NEOMYCIN 3.5 MG-POLYMYXIN 10,000 UNIT-HYDROCORT 10 MG/ML EYE DROP,SUSP

  • TRAVOPROST 0.004 % EYE DROPS

  • BETAXOLOL 0.25 % EYE DROPS,SUSPENSION

  • CIPROFLOXACIN 0.3 % EYE DROPS

  • GATIFLOXACIN 0.5 % EYE DROPS

  • AZITHROMYCIN 1 % EYE DROPS

  • GENTAMICIN 0.3 % EYE DROPS

  • SULFACETAMIDE SODIUM 10 % EYE DROPS

  • FLUOROMETHOLONE 0.1 % EYE DROPS,SUSPENSION

  • FLUOROMETHOLONE 0.25 % EYE DROPS,SUSPENSION

  • PREDNISOLONE ACETATE 0.12 % EYE DROPS,SUSPENSION

NSAIDs Removed:

  • Naproxen 375 mg tablet

    • We have naproxen 250 mg tablets

  • Aspirin-dipyridamole 25 mg-200 mg extended release capsule

  • Aspirin 600 mg suppository

    • We have 300 mg suppositories

  • Ketorolac 10 mg tablets

  • Etodolac 200 mg capsules

  • Nabumetone 500 mg tablet

  • Sulindac 150 and 200 mg tablet

DPP-IV Inhibitors: No changes

Benzodiazepines for Seizures Removed:

  • Diazepam 2.5 mg rectal kit

Otic Fluoroquinolones Added:

  • Ciprofloxacin 0.3%-dexamethasone 0.1% otic solution

  • Please note that there are classes of medication that we haven't yet harmonized in Epic, but we will continue this process over the next few months. Please use Epic as the best resource for what we currently carry during this transition time.

  • The LMA will push the provider to the formulary agent and for medications like ACEi, will include a table with equivalent dose but that dose doesn't automatically populate in Epic. Double-check dosing during verification.
